Odontogenic Cysts of upper jaw an analysis

This article attempts to analyze all cases of odontogenic cysts involving upper jaw who presented at
Stanley Medical college during 2007 – 2012. This article analyzes the incidence of these cysts
during the above said period, age of occurence, sex prediliction if any, clinical presentations and
optimal treatment modality. Common complaints with which patients presented to our Institution
was swelling over jaw, next was loosening of dentition, paresthesia. 30 patients had presented with
cysts involving upper jaw out of which 29 were females and one was male. All these patients
underwent surgical removal of the cystic lesion.

We report a case of primary laryngeal amyloidosis in a 35 year old adult
patient who presented with hoarseness of voice for 6 months duration with no other
symptoms. Patient was treated successfully with endoscopic excision using a
microdebrider with a laryngeal blade and review of literature.

CORRELATION BETWEEN NEURAL RESPONSE TELEMETRY (NRT) MESUREMENT LEVEL AND BEHAVIORL (T-LEVEL AND C-LEVEL) IN PRELINGUAL COCHLEAR IMPLANT PATIENTS

Objectives: To study the correlation between Neural Response Telemetry (NRT)
measurement level and behavioral (Threshold level and Comfort level) in pre lingual
cochlear implant patients age between 2 -10 years old at one and three months post implant.
Methods: A cross sectional study conducted at University Kebangsaan Malaysia Medical
Center from September 2010 to January 2012. Total numbers of hundred patients were
involved in this study.
All recipients implanted with Nucleus 24 cochlear implant and had full insertion and normal
activation of the electrode array. Comparison between intra-operative NRT measurement
level and behavioral (T-level and C-level) in cochlear implant patients at one month and
three months post implantation were obtained respectively.
Results: This study showed the intra-operative NRT levels were seen to fall between the T
and C levels in one and three months respectively. There was also a positive correlation
between NRT value measurements and both T and C value measurements in both one and
three months (p value 0.01). There is a fair strength of the linear relationship between NRT
and behavioral level in both one and three month post implant as shown by the r value (0.4 at
one month, 0.2 at three months)
Conclusion: It is useful to use the NRT values to predict the behavioral T and C values in
prelingual children and an additional tool for the mapping.
A STUDY ON EFFICACY OF AUTOLOGOUS PLATELET RICH PLASMA IN MYRINGOPLASTY

Objective: To study the efficacy and advantage in using autologus platelet rich
plasma during myringoplasty in closure of tympanic membrane perforation
preventing the graft displacement, promoting quicker healing & improving overall
outcome.
Methods : Platelet rich plasma(PRP) is an autologus platelet rich concentrate
prepared from patients own blood with growth factors up to 8 times that of normal
serum and its efficacy when used during myringoplasty is studied.50 patients with
chronic otitis media inactive mucosal disease were randomly chosen and 25 of
them was the study group and other 25 were the control group. Both group patients
underwent myringoplasty and PRP was used in the study group and the results
were evaluated.
Results: In our study among 25 cases that underwent myringoplasty with use of
platelet rich fibrin, 24 had complete tympanic membrane closure and only one
failure has been noticed. In controls 5 out of 25 cases had failure. The graft take up
rate in our study is comparable with the reference studies. Use of PRP accelerates
graft uptake.
Conclusion: Platelet rich plasma is a cheap and cost effective platelet concentrate
with enriched growth factors. It accelerates the tympanic membrane closure
following myringoplasty.

Crouzon’s syndrome with adenotonsillitis: conventional surgery in altered anatomy.

Background/Objectives: Crouzon’s syndrome is characterized by premature closure of the
cranial sutures, midface hypoplasia, orbital deformities & other associated abnormalities.
Children with Crouzon syndrome frequently have obstructive sleep apnea due to the
underdevelopment of the midface.
Case report: A 12 year old boy of Crouzon’s syndrome with chronic adeno-tonsillitis was
managed by adeno-tonsillectomy under general anaesthesia by scalpel cautery method. The boy
responded well to surgery & the mild sleep disorder disappeared within a week uveventfully.
Conclusion: Sleep disorders in this condition can be treated by improving the airway by
selective procedures like midface advancement, mandibular expansion , adeno-tonsillectomy,
uvulo-palatopharyngoplasty, anterior tongue reduction & endoscopic tracheal granuloma
excision.
Neck Space infections A Case series study

This article is about the cases of neck space infections presented to the department of E.N.T and
HEAD & NECK surgery of Govt.stanley medical college, during a two month period of 15th
November 2011to 15th January 2012. Out of the 8 cases presented to the department, 2 cases were
of tuberculous cold abscess. Five cases treated with I&D; of which two cases needed preliminary
tracheostomy.

Aims and background
The aim of our research was to use lymphoscintigraphy as a main method to confirm and detect
lymph nodes in the neck, in patients with squamos cell carcinoma of lower lip which were clinically
T1, T2 and N0, and to justify the use of selective neck dissection in those patients.
Methods
From April 2010 to January 2011, 31 patients with T1, T2 and N0 SCC of the lower lip were
admitted to our center. To detect sentinel lymph nodes, we performed lymphoscintigraphy (LSG).
LSG was performed on the day of surgery after intradermal injection of 37 Mbq of
Tc99m-Sn-colloid/ml at four peritumoral sites. The sentinel lymph nodes were then extirpated and
sent for biopsy.
Results
Among the 31 patients, three (9.7%) were female and 28 (90.3%) were male. LSG detected sentinel
nodes in the neck in 21 (67.7%) of the patients. Of these, 10 (47.6%) had a positive sentinel node
biopsy. Of all 31 patients enrolled in the study, occult metastases were found in 10 (32.3%).
Conclusions
Our results indicate that, of the methods used to detect positive lymph nodes, the most accurate is
LSG. The results also suggest that further study is needed to optimize the treatment protocol in
patients with SCC of the lower lip, especially in those with T2 lesions.
